Beyond Compliance: Preventing Serious Injuries and Fatalities (SIFs)

Despite overall injury rates declining, serious injuries and fatalities (SIFs) continue to occur each year—often with life-altering consequences. Traditional safety metrics alone are not enough to prevent these incidents. This session explores a focused, risk-based approach to SIF prevention and offers practical strategies to better protect workers in construction and industrial environments.

Objectives:

  1. Define Serious Injuries and Fatalities (SIFs) and distinguish them from other workplace incidents.
  2. Understand the common precursors and high-energy hazards that contribute to SIF events.
  3. Identify proactive methods to integrate SIF prevention into safety management systems.
  4. Explore real-world lessons learned and actionable steps to drive SIF prevention in the workplace.

Mark Maynard, MBA, CSP, CHST, CPEA, CertIOSH – Director, Construction Safety with STACK Infrastructure

Mark Maynard provides Construction Safety support to their data center projects in North America, as well as leads implementation of their Serious Injury & Fatality (SIF) Prevention Program with their contractors and vendors. With over 37 years of experience in health, safety, security, and environmental management, Mark specializes in high-hazard industries, including large-scale construction, heavy equipment operations, DoD, and DOE projects. He is a Board-Certified Safety Professional, Certified Member of IOSH, and Authorized OSHA Outreach Instructor with expertise in Serious Injury and Fatality (SIF) prevention, and regulatory compliance. Mark has led initiatives to implement risk-based SIF prevention programs across diverse construction environments, helping organizations and trade partners reduce exposures and strengthen their safety culture.
